Tirzepatide: canlyniadau'r treialon

SURMOUNT-1: y maint effaith mwyaf yn y maes

Rhoes SURMOUNT-1 oedolion â gordewdra ac heb ddiabetes ar tirzepatide 5, 10 neu 15 mg bob wythnos, neu placebo, am 72 wythnos [8]. Roedd y newid pwysau cyfartalog ar y dos uchaf tua −20.9%, gyda'r dos 5 mg eisoes yn oddeutu −15% — sy'n golygu bod y dos tirzepatide isaf yn cyfateb yn fras i'r canlyniad gorau semaglutide. Roedd mwy na hanner y cyfranogwyr ar y dosau uwch wedi colli o leiaf 20% o bwysau'r corff.

Trechu semaglutide yn uniongyrchol

Roedd cymariaethau anuniongyrchol a chohortau byd go iawn wedi cyfeirio yma am gyfnod [12], ond y treial a sefydlodd hyn oedd y gymhariaeth benben rhwng y ddau feddyginiaeth ar ddosau gordewdra yn yr un poblogaeth, a chafodd tirzepatide ei fuddugoliaeth ar newid pwysau cyfartalog gyda chymharol fawr [10]. Mewn diabetes math 2, roedd SURPASS-2 eisoes wedi dangos gwelliant dros semaglutide 1 mg ar y ddau bwynt terfyn glycaemig a phwysau [9].

Y canlyniadau nad ydynt yn ymwneud â phwysau

Symudodd dau dreial neilltuol tirzepatide y tu hwnt i gordewdra fel categori.

  • Apnoea cwsg obstructive. Mewn oedolion â OSA cymedrol i ddifrifol a gordewdra, lleihawyd yr index apnoea-hypopnoea yn sylweddol yn erbyn placebo [6]. Dyma'r driniaeth feddygol gyntaf gyda thystiolaeth wedi'i neilltuo mewn cyflwr a reolwyd bron yn gyfan gwbl gyda CPAP.
  • Clefyd y galon gyda ffrithiant eithafol cadwraeth. Mewn HFpEF gyda gordewdra — phenoteip gyda phrofiadau effeithiol ychydig iawn — gwellaodd tirzepatide ganlyniadau yn erbyn placebo [4].
  • Digwyddiadau cardiofasgwlaidd. Cafodd cymhariaeth neilltuol yn erbyn dulaglutide ei chofnodi ar ganlyniadau cardiofasgwlaidd mewn diabetes math 2 [11], gan ychwanegu at y meta-analys cyn-gynhelir a ddarganfyddodd nad oedd unrhyw signal o niwed cardiofasgwlaidd.

Mae hefyd yn ymddangos yn y cyfarwyddyd Ewropeaidd ar glefyd yr afu steatotig ynghyd â semaglutide [2].
