GLP-1 zāles un ceļa osteoartrīts: pētījuma rezultāti

Svara zaudēšana ceļa locītavas osteoartrītam vienmēr ir tikusi ieteikta, un zāļu klase, kas uzticami rada 15–20% ķermeņa svara zudumu, agri vai vēlu bija jāpārbauda arī šajā jomā. Tagad viens šāds pētījums ir veikts. Lūk, ko tas atklāja un ko nē.

Kāpēc šis jautājums ir pamatots

Ceļa locītavas osteoartrīts nav tikai mehānisks nodilums, taču mehāniskā slodze ir daļa no ainas, un mazāks ķermeņa svars nozīmē mazāku slodzi uz svaru nesošu locītavu — tas ir sen zināms ortopēdisks pamatojums, nevis GLP-1 pētījumu atklājums. Aptaukošanās ar osteoartrītu ir saistīta arī caur vielmaiņas un iekaisuma mehānismiem, kas pārsniedz vienkāršu slodzi — taukaudi ir vielmaiņas ziņā aktīvi, un sistēmisks iekaisums ir ticams otrs ceļš, caur kuru svara zaudēšana varētu palīdzēt pat locītavām, kas nemaz nenes svaru, piemēram, rokām. Zāles, kas rada gan būtisku svara zudumu, gan, saskaņā ar šīs klases mehānistiskajiem pārskatiem, izmaiņas apetītes un enerģijas regulācijā caur centrālajiem un perifērajiem mehānismiem, ir pamatots kandidāts pārbaudei [37].

Pētījums

Randomizētā, placebo kontrolētā pētījumā tieši cilvēkiem ar aptaukošanos un ceļa locītavas osteoartrītu pārbaudīts reizi nedēļā lietojams semaglutīds [17]. Šī ir cita populācija un cits primārais mērķa kritērijs nekā citviet šajā vietnē aplūkotajos semaglutīda un tirzepatīda aptaukošanās pētījumos: šeit mērītie iznākumi bija osteoartrīta sāpes un fiziskā funkcija, nevis tikai svars. Pētījumā ziņots par lielāku uzlabojumu semaglutīda grupā nekā placebo grupā gan sāpju, gan funkcijas rādītājos, papildus svara zudumam, kas šīm zālēm konsekventi novērots citos pētījumos. Tas ir patiesi jauna veida rezultāts šai zāļu klasei — randomizēts pētījums ar locītavai specifisku mērķa kritēriju, nevis secinājums, kas izdarīts no svara zaudēšanas pētījumiem, kuros gadījuma kārtā pajautāts arī par sāpēm kā sekundāru rādītāju.

Šī trešā rinda ir svarīga, un to šī pētījuma atspoguļojumos viegli palaist garām. Beztauku masa — tostarp muskuļi, kas stabilizē ceļa locītavu — veido aptuveni ceturtdaļu līdz trešdaļu no svara, kas zaudēts, lietojot šīs zāles; šo atklājumu vietne sīkāk aplūko savā muskuļu zuduma lapā. Osteoartrīta pētījuma kopējais rezultāts tomēr bija pozitīvs gan sāpju, gan funkcijas ziņā, kas nozīmē, ka zaudētie muskuļi nepārsniedza slodzes samazinājuma un iespējamā pretiekaisuma efekta ieguvumu. Taču tas nozīmē, ka godīgs šo zāļu ietekmes apraksts uz locītavu ir "divu pretēji vērstu procesu kopsumma", nevis vienkārši "svara zaudēšana palīdz locītavām".

Ko šis pētījums neatklāj

Viens pētījums ar vienām zālēm, cilvēkiem, kuriem iekļaušanas brīdī bija aptaukošanās un diagnosticēts ceļa locītavas osteoartrīts, nepierāda efektu visai zāļu klasei, citām locītavām, cilvēkiem bez aptaukošanās vai ilgtermiņa strukturāliem iznākumiem, piemēram, skrimšļa apjomam. Tas arī neatklāj, cik ilgstošs ir ieguvums pēc zāļu lietošanas pārtraukšanas — uz šo jautājumu attiecībā uz pašu svaru šīs vietnes pārtraukšanas lapa jau atbild nelabvēlīgi, taču nav atsevišķu osteoartrīta turpinājuma pētījumu datu, kas norādītu, vai locītavu sāpju mazināšanās seko tam pašam atgriešanās modelim.

Ko tas nozīmē praksē

Ja ceļa sāpes un liekais svars pastāv vienlaikus, tas tagad ir vēl viens pierādījums — konkrēts, randomizēts — ka GLP-1 zāļu izraisītais svara zudums var izpausties kā mērāms ieguvums locītavai, nevis tikai skaitlis uz svariem. Tas nav pierādījums, ka šīs zāles ārstē osteoartrītu neatkarīgi no svara zaudēšanas, un tas nav pierādījums citām locītavām vai citām populācijām. Blakusparādību profils un muskuļu zuduma jautājums, kas attiecas uz jebkādu šīs zāļu klases lietošanu, attiecas arī šeit; neviens no tiem nemainās tāpēc, ka mērķa iznākums ir locītava, nevis vidukļa apkārtmērs.

Biežāk uzdotie jautājumi

Vai semaglutīds ārstē osteoartrītu, vai tas palīdz tikai ar svara zaudēšanu?
Pētījums šos divus aspektus nenošķir. Tajā zāles salīdzinātas ar placebo, mērītas sāpes un funkcija, un konstatēts ieguvums; tas nepārbaudīja, vai efekts pastāv neatkarīgi no svara izmaiņām [17].
Vai zāles izraisa muskuļu zudumu tieši ap ceļa locītavu?
Publicētie ķermeņa sastāva dati mēra kopējo beztauku masu, nevis muskuļus konkrētā locītavā, tāpēc precīzi atbildēt nevar — taču tie liecina par nozīmīgu kopējo beztauku masas zudumu ārstēšanas laikā [3].
Vai šis pētījums attiecas tikai uz semaglutīdu, vai arī uz tirzepatīdu?
Pagaidām šajā konkrētajā populācijā un ar šo mērķa kritēriju pārbaudīts tikai semaglutīds. Tirzepatīda lielākais vidējais svara zudums (skatiet tirzepatīda lapu) ir pamatots iemesls sagaidīt līdzīgu vai lielāku efektu, taču tā ir ekstrapolācija, nevis pētījuma pierādījums.
Ja pārtraucu lietot zāles, vai ieguvums locītavai izzūd kopā ar svaru?
Par šo jautājumu nav atsevišķu ilgtermiņa turpinājuma pētījumu datu. Ņemot vērā, cik konsekventi svars atgriežas pēc lietošanas pārtraukšanas (skatiet kas notiek, kad pārtraucat), būtu pamatoti sagaidīt, ka locītavas simptomi seko svaram, taču tas nav tieši mērīts.
