GLP-1-medicijnen en knieartrose: wat het onderzoek vond

Gewichtsverlies wordt al lang aanbevolen bij knieartrose, en een geneesmiddelklasse die betrouwbaar 15–20% lichaamsgewichtsverlies oplevert, zou daar vroeg of laat op getest worden. Inmiddels is er één trial geweest. Dit is wat die vond, en wat niet.

Waarom de vraag logisch is

Knieartrose is niet puur mechanische slijtage, maar mechanische belasting maakt wel deel uit van het beeld, en minder lichaamsgewicht betekent minder belasting op een gewichtsdragend gewricht — een langbestaande orthopedische redenering, geen bevinding uit de GLP-1-trials. Obesitas wordt ook in verband gebracht met artrose via metabole en ontstekingsroutes die verder gaan dan eenvoudige belasting — vetweefsel is metabool actief, en systemische ontsteking is een plausibele tweede route waarlangs gewichtsverlies gewrichten kan helpen die helemaal geen gewicht dragen, zoals de handen. Een geneesmiddel dat zowel aanzienlijk gewichtsverlies oplevert als, volgens mechanistische reviews van deze klasse, veranderingen in eetlust en energieregulatie via centrale en perifere routes, is een redelijke kandidaat om te testen [37].

De proef

Een gerandomiseerde, placebogecontroleerde trial testte semaglutide eenmaal per week specifiek bij mensen met obesitas en knieartrose [17]. Dit is een andere populatie en een ander primair eindpunt dan de semaglutide- en tirzepatide-obesitastrials die elders op deze site worden behandeld: de hier gemeten uitkomsten waren artrosepijn en fysiek functioneren, niet alleen gewicht. De trial rapporteerde een grotere verbetering in de semaglutidegroep dan in de placebogroep op zowel pijn- als functiematen, naast het gewichtsverlies dat consistent wordt gezien met dit geneesmiddel in andere trials. Die derde rij is belangrijk en wordt gemakkelijk over het hoofd gezien in berichtgeving over deze trial. Vetvrije massa — inclusief de spieren die een kniegewricht stabiliseren — maakt ongeveer een kwart tot een derde uit van het gewicht dat met deze geneesmiddelen wordt verloren, een bevinding die deze site in detail behandelt op de pagina over spierverlies. Het netto resultaat van de artrosetrial was nog steeds positief voor pijn en functie, wat betekent dat het verloren spierweefsel niet opwoog tegen het voordeel van de verminderde belasting en het mogelijke ontstekingsremmende effect. Het is geen bewijs dat deze geneesmiddelen artrose behandelen onafhankelijk van gewichtsverlies, en het is geen bewijs voor andere gewrichten of andere populaties. Het bijwerkingenprofiel en de vraag over spierverlies die gelden voor elk gebruik van deze geneesmiddelklasse, gelden ook hier; geen van beide is anders omdat de beoogde uitkomst een gewricht is in plaats van een taille.

Behandelt semaglutide artrose, of helpt het alleen via gewichtsverlies?
De trial maakt geen onderscheid tussen de twee. Het geneesmiddel werd getest tegen placebo en pijn en functie werden gemeten, en er werd een voordeel gevonden; er werd niet getest of het effect bestaat onafhankelijk van gewichtsverandering [17].
Veroorzaakt het geneesmiddel specifiek spierverlies rond de knie?
De gepubliceerde gegevens over lichaamssamenstelling meten de totale vetvrije massa, niet spieren bij een specifiek gewricht, dus kunnen ze die vraag niet precies beantwoorden — maar ze tonen wel een significant verlies van vetvrije massa over de gehele behandeling [3].
Gold deze trial alleen voor semaglutide, of geldt dit ook voor tirzepatide?

Er zijn geen specifieke langetermijn-follow-upgegevens over die vraag. Gezien hoe consistent het gewicht terugkeert na het stoppen (zie wat er gebeurt als je stopt), zou het redelijk zijn te verwachten dat gewrichtsklachten het gewicht volgen, maar dit is niet direct gemeten.
