GLP-1 drugs beyond weight loss: kidneys, liver, sleep apnoea, addiction

Some of the non-weight findings are as solid as the weight data. Others are one small trial and a great deal of enthusiasm. Here is which is which.

Kidneys: the strongest non-weight case

A 2025 meta-analysis of randomised trials examined kidney and cardiovascular disease outcomes together and found consistent benefit [1]. In people who already have chronic kidney disease, a focused systematic review reached the same conclusion [2]. Earlier pooled analyses had already flagged the kidney signal alongside the cardiovascular one [5][10], and tirzepatide showed kidney benefit against insulin glargine in the SURPASS-4 analysis [7].

The relationship between obesity and kidney disease is itself well characterised [9], so a weight-mediated pathway is plausible — but as with cardiovascular outcomes, the trials cannot separate direct from weight-mediated effects.

Liver: into the guidelines

Metabolic dysfunction-associated steatotic liver disease is the area where non-weight use has moved fastest from trial to practice. Tirzepatide reduced liver fat content and abdominal adipose tissue against insulin degludec in a randomised comparison [6], and semaglutide has accumulated enough evidence to feature in formal European management guidance. A network meta-analysis of diabetes drug treatments places this class favourably on the broader benefit-harm balance [4].

Sleep apnoea

Covered in full on the tirzepatide page: a dedicated randomised trial in moderate-to-severe obstructive sleep apnoea with obesity produced a substantial reduction in the apnoea-hypopnoea index. This is the first drug with randomised evidence in a condition that has otherwise been the exclusive domain of CPAP and surgery.

Addiction: the interesting one, and the weakest

The mechanistic story is attractive. GLP-1 receptors are present in brain reward circuitry, the drugs reduce food-related craving, and users spontaneously report drinking less alcohol. If GLP-1 signalling modulates reward generally rather than food reward specifically, effects on other addictive behaviours would follow.

The human evidence has not caught up. A randomised placebo-controlled trial of exenatide in alcohol use disorder did not produce a clean positive primary result, though it reported signals in subgroups and imaging endpoints [8]. That is a reasonable basis for further trials and a poor basis for prescribing.

Addiction and substance use

What we are deliberately not covering yet

Neurodegeneration, polycystic ovary syndrome, chronic inflammation and cancer risk all have active trial programmes and essentially no completed randomised evidence in the populations being discussed online. When those trials report, this page will change. Until then, treating them as established is exactly the error that makes health writing untrustworthy.

Common questions

Will it help my drinking?
Possibly, but the one randomised trial did not show a clear benefit on its primary endpoint [8]. Anecdote is currently ahead of evidence.
Can I take it for kidney protection?
The kidney evidence is strong [1][2] and this is increasingly part of clinical guidance in diabetes and chronic kidney disease. It is a conversation to have with a nephrologist.
Does it reduce inflammation?
Inflammatory markers improve, but that is expected with substantial weight loss and does not by itself establish a clinical benefit.
Is it useful for PCOS?
Weight loss helps PCOS, and these drugs cause weight loss. Dedicated randomised evidence with PCOS-specific endpoints is limited.
