Semaglutida: lo que realmente muestran los ensayos
Agonista del receptor GLP-1. Inyección semanal; también existe una forma oral diaria.
Lo que la evidencia apoya
STEP 1 informó una reducción media del peso corporal de aproximadamente 15% a las 68 semanas frente a aproximadamente 2,4% con placebo. SELECT informó posteriormente una reducción en los eventos cardiovasculares adversos mayores en personas con sobrepeso y enfermedad cardiovascular establecida pero sin diabetes. [1]
Los efectos gastrointestinales son comunes y son la razón habitual por la que las personas interrumpen el tratamiento. La recuperación de peso tras la interrupción es sustancial y está bien documentada: STEP 4 mostró que la mayor parte de la pérdida regresaba dentro de un año después de dejar el tratamiento.
Qué sucede cuando se interrumpe
Esta es la parte de la imagen que más a menudo se omite. Los ensayos de retirada muestran consistentemente una recuperación de peso sustancial dentro de un año de la interrupción, y los marcadores cardiometabólicos regresan con ello [2]. Eso no es un fallo del fármaco: es lo que implica tratar una condición crónica con un fármaco cuando este se retira.
La implicación práctica es que la decisión de comenzar es una decisión sobre un horizonte largo, incluyendo el costo y el acceso durante ese horizonte, más que sobre un peso objetivo.
Masa muscular
La pérdida rápida de peso por cualquier causa incluye tejido magro, y los ensayos de incretinas no son la excepción. La proporción perdida como masa magra parece ser en general similar a otros métodos de pérdida rápida, pero la pérdida absoluta es mayor porque la pérdida total es mayor. Una ingesta adecuada de proteínas y el entrenamiento de resistencia son las intervenciones con evidencia que las respalda, y ambas están bajo su control mientras toma el fármaco.
