O que a evidencia apoia
A xeración anterior. O SCALE informou dunha redución media de peso de aproximadamente 8% a 56 semanas — real, e claramente menos que os axentes semanais que seguiron. [1]
A inxección diaria e o efecto menor fixeron que sexa en gran medida unha segunda opción onde os fármacos semanais están dispoñibles. Segue sendo relevante onde non o están, ou onde o custo difire.
Que sucede cando paras
Esta é a parte da imaxe que máis a miúdo se omite. Os ensaios de retirada mostran consistentemente unha recuperación de peso substancial dentro dun ano de parar, e os marcadores cardiometabólicos volven a aumentar [2]. Iso non é un fallo do fármaco — é o que implica tratar unha condición crónica cun fármaco cando este se retira.
A implicación práctica é que a decisión de comezar é unha decisión sobre un horizonte longo, incluíndo custo e acceso durante ese horizonte, en lugar de sobre un peso obxectivo.
Masa muscular
A perda de peso rápida por calquera causa inclúe tecido magro, e os ensaios de incretinas non son unha excepción. A proporción perdida como masa magra parece ser amplamente similar a outros métodos de perda rápida, pero a perda absoluta é maior porque a perda total é maior. Unha adecuada ingesta de proteínas e o adestramento de resistencia son as intervencións que contan con evidencia detrás delas, e ambas están dentro do seu control mentres toma o fármaco.
